Open the form by clicking on the downloaded file
Print the form into an A4 size paper, verify that
it is printed in exactly the same manner as it appears

Mention your sub broker code (Allotted and applicable
only to BCL Sub Brokers) in the sub broker
stamp/code column
Carefully read the instructions printed on the form.
Fill the printed form (As normally filled) and attach
the cheque/ DD. |
